This fee is an access/membership fee that includes only the services listed below. THIS MEMBERSHIP IS NOT INSURANCE, it does not take the place of insurance, however we believe it is an affordable adjunct to your insurance with immense value in helping you get the best out of healthcare options available today. Some medical services may be reimbursable as an out of network expense with your insurance plan, or through your HSA/FSA. We do not bill your insurance and we are not in network with any insurance plans including medicare. For medical services that may be reimbursable through your insurance, we offer a printed invoice and copy of your progress note which you can submit to the insurance for reimbursement.


Monthly Concierge Membership Services

  • Annual Executive Comprehensive Physical

  • 1 medical visit/month. (health discussion, physical exam, assessment as well as ordering necessary tests, medications, specialty consultations, therapies etc and providing empowering education about your diagnoses) toward your primary health goals,

  • 1 wellness visit/month, select from the a la carte wellness menu and scheduled in advance. Can be combined with your medical visit for your convenience if you like. Look forward to these exquisite health rituals that help you get excited about living your healthiest lifestyle.

  • Timely Acute Visits for acute illness or needs

  • Quarterly extended visits focused on managing complex or chronic disease if indicated

  • Timely Pre-Operative Surgical Clearance visits

  • Hospital/ER followup visits, wherever possible within 72 hours of discharge

  • Advanced Care Planning and support for sorting out your advanced directive, living will, medical power of attorney designations so they are ready before you need

  • Tele-medicine visits

  • HIPPA-Secure Portal Access and maximum HIPPA-Secure Communications at all times

  • Basic yearly labs. This includes a yearly CBC with differential, CMP, Lipid panel if indicated, Urinalysis. Additional labs indicated for your specific situation will incur separate charge however discounts are offerred and often these expenses can be run through your insurance or FSA/HSA account.

  • Hands on care coordination with subspecialists, surgeons and ancillary healthcare team

  • Need to get your medications straight? We will perform a detailed medication reconcilliation, making sure you are aware of any possible drug interactions or side effects that could be impacting you. Looking for ways to safely limit the number of pills you take, as well as a special look at how to lower the cost of the pills you take

  • Planning an international trip? We've got you covered. Meet beforehand to learn about special considerations to lower your risk of becoming ill while traveling, get your prescription for any prophylactic or preventive medications you may need, get any necessary vaccines ordered.

  • After hours availability on a case-by-case basis and arranged in advance for the convenience of the patient at a time that is mutually available for patient and doctor, though we do not provide Emergency Room care.

  • Basic yearly labs. This includes a yearly CBC with differential, CMP, Lipid panel if indicated, Urinalysis.   • High touch care coorination with wrap around services to support your healthiest lifestyle as needed: Home Health, Palliative Care, Hospice, Medical Equipment/Supply Companies, Home Oxygen, In home lab draws, Pharmacy services including vaccines, compounded medications, and infusions

  2. ​​​*While we intentionally keep a small panel of patients in order to have the availability to serve our clients timely and in the discrete, detailed and often time intensive manner luxury healthcare demands, fee for service appointments are on a limited availability, schedule permitting basis as scheduling priority goes to clients who have purchased monthly and yearly memberships.

*All checkmarked services with a single * are available on a pay-as-you go hourly rate basis, and are available in limited supply on a first come, first serve basis. Pro-rated hourly charges will be applied to portal and text messaging in 15 minute intervals.

**As part of the 1 medical visit/month included in the plan. Visits in excess of this incur the hourly rate charge. Visits not used by the end of the month do not rollover to future months.

****This does not count toward your 1 medical visit/month. Our goal is to keep you out of the ER and hospital, but in the case it can not safely be prevented, this visit is on us and intended to decrease the likelihood of a repeat admission.

^As part of the 1 wellness visit/month included in the plan. Wellness visits are those included on the Wellness Menu in the a la carte option. Wellness visits in excess of once a month are available upon request where space permits.

^^ Wherever possible, we will seek to accomodate these within 72 hours of the initial request. This includes EKG, Chest Xray, pre-operative labs as indicated, interpretation of all results and discussing and sharing all results with your surgeon or necessary subspecialists as well as referrals and care coordination to any additional specialist you may need to see to optimize your health and lower your risk of surgical complications. It also includes a telehealth checkin at week 1 and 2 toward the goal of helping you manage your non-surgical conditions post-opearatively. It does not include post operative pain management or other surgical specific needs which should be discussed and managed by your surgeon.

^^^Dr. Tequa Salehi-rad will accompany you, by request, to your local sub-specialist or surgeon visit with advanced schedule coordination toward the goal of optimizing care coordination and serving as your healthcare advocate. Travel outside of the local area to accompany you to subspecialists or surgeons available on special request and requires advance schedule coordination and subject to additional travel fees.

^^^^Travel outside of the local area to advocate for you during your hospital stay is limited based on doctor availability and subject to additional travel fees. Even at local hospitals, the doctor's presence is as an advocate, like a professional family member, toward the goal of looking out for your best health interest, but not able to make decisions for you in the hospital or place orders on your behalf. When you are in the hospital, the hospital team makes the final decisions and places the orders for your inpatient care. That said, often those conversations at bedside between your hospital team and your concierge doctor can improve the quality of care you receive, prevent medical errors, drive optimal decision making, reduce costs for unnecessary testing, shorten your hospital stay and give you peace of mind and support when you need it most!
